5 Minute NOTAM usage survey for UK Pilots

I am a GA pilot and the provider of the fly.dsc.net website. I am interested to hear your opinions on the past and current situation for the provision of NOTAMs. There is a lot of discussion on this topic on some websites and elsewhere, but there is little hard data as to who checks NOTAMs and how they do it. To this end, in consultation with others, I've created this survey.
This survey is completely anonymous. You don't have to enter a name, email address or any other such information. Your IP address is not being logged against your answers.
Please try to answer all the questions that are applicable to you and click the Submit button at the end. In common with most surveys, there might not be a perfect answer to describe your exact situation - in this case, please just choose the closest option. You can use the comments section at the end to provide further explanation if you wish.
